Services Available From Stewartville Home Care Services
(Effective August 1, 2008)
General Information:
For your convenience, our homecare services are offered as a package which include
rent, meals and home care services as described below. Any person(s) requesting
homecare services will have an individualized case mix assessment to be completed
by the homecare nurse to determine level of care. The cost for your services will be
based on your level of care. Packages range from $2,349.00 - $3,973.00 per month
and is paid at the beginning of the month for that month’s service. A care plan is
developed by the homecare nurse to ensure that your needs are met. If the care plan
is agreed upon by both parties, a service agreement will be signed outlining services
and you will be provided with a copy. The above will be reviewed and updated
annually or when there is a major change in functional status or care needs.
Assisted Living Packages May Include:
Monthly rent
All utilities, except cable TV and telephone
3 Meals per day served in the dining room
Daily well being checks (optional participation)
24 hours per day emergency response
Limited activities (Activity Packages may be available)
Local scheduled transportation
RN individualized assessment and care plan development / Admission to Home Care
Staff assist in setting up appointments / transportation where needed
Wellness Clinics – Bi-monthly BP & WT screenings
Care / Home Health Aide:
Assisting with morning and/or bedtime care, personal care, bathing, toileting, assisting with mobility, range of motion and escorting. The HHA may also monitor BP, pulse and/or weight, or assist with the administration of eye or ear medication and other delegated medications / functions following instruction by the RN.
Care / Home Care Aide: (Home Management Tasks):
Laundry, linen change, light housekeeping (mopping, dusting, vacuuming, bathroom/kitchen duties), grocery shopping, errands escorting, companionship or supervision, meal/snack prep. Cleaning supplies/laundry detergent are included in package cost.
Care / Licensed Nurse: Assessments, medication set-up, wound care; nail care, lab management and/or other special treatments.
Additions to Package: Any additions to agreed upon package are billed individually above package rate.
Home Health Aide / $9.53 per 15 minutes – Care or Supervision
Licensed Nurse Services / $18.43 per 15 minutes
Escort out of building / $24.09 per hour
Foot Care / $18.43
Housekeeping/Laundry / $9.53 per 15 minutes
Medication Administration/Assist/Reminders-by HHA / $4.67 per time
Escort / $381.30 per month
Medication Set-up / $36.86 per time
Meal Delivery to Apartment / $4.67 per time
Transfer Assist (2nd person) / $4.67 per time
Use of Emergency Pendant to summon non-emergency assist may be charged
$42.11 per occurrence. (There is no charge to use the emergency pendant to summon
emergency assist 24 hours per day.)
** Laundry > than 2 loads per week
** Shower > than 1 time per week
** Housekeeping > than 90 minutes per week
** Other Services not listed in Assisted Living Package agreement
** Supplies other than housekeeping supplies
We are licensed with the State of Minnesota. All rates are subject to change with a 30 day
notice. Services are individually tailored to meet your needs. For questions or further

** We do not offer ala-carte home care services with the exception of foot care,
housekeeping and/or meals. All other services must be purchased as an Assisted
* Married couples who both desire services will each have an individualized assessment,
case mix level and careplan. Meals and Home Care Services willed be billed per
person and the Rent portion will be billed to one spouse.
* Please note that it is our policy that no medication reminders, assist or administration
will be done unless SHCS has set up the medications.
* Arrangements can be made for physical, occupational or speech therapy.
ASSISTED LIVING PACKAGES

level of services needed. A care plan is developed by the nurse to ensure
that your needs are met. It is updated annually or when there is a major

Meal prep, laundry, linen change, grocery shopping, light housekeeping
(mopping, dusting, vacuuming, bathroom/kitchen duties), errands,
escorting, companionship or supervision, --Cleaning supplies/
laundry detergents are included in the package cost—
Assisted living package rates:
Range from $2,334.00-$3973.00 per month as determined by care, needs and
case mix assessment. It is paid at the beginning of each month for that
month’s service.
Additions to package:
* Foot care, escorting outside of Root River Estates, showers > than once
per week, laundry > than 2 loads per week, supplies (other than house-
keeping supplies), or other services not listed in Assisted Living package are
available at additional cost.
